Dr. Butuk has been practicing family medicine since 1994. He graduated from medical school in 1991 and completed his family practice residency in 1994. Dr. Butuk is board certified by the American Board of Family Practice and is a member of the American Board of Family Physicians and the Idaho Medical Association.

Dr. Butuk established Meridian Family Medicine in 2006 to create a doctor-owned medical practice with the ability to provide every patient with first-rate medical care. Since 2006, Dr. Butuk has added five medical providers to join the Meridian Family Medicine medical staff. He enjoys treating patients of all ages and the whole family. Dr. Butuk focuses on preventive medicine with an emphasis on healthy nutrition and lifestyle.

Dr. Butuk, his wife, and two children have made Meridian their home since 2003 and have enjoyed all the benefits of living in Idaho. He enjoys a busy and active lifestyle outside the clinic.

Scott comes to Meridian Family Medicine with over twelve years of experience in healthcare, focused primarily in urgent care and family practice.

A graduate with a Master's of Physician Assistant Studies from Rocky Mountain College, he worked as the general surgery team leader in Bozeman, Montana before moving to Utah and overseeing the diagnosis and management of acute and chronic conditions for all age groups. Eventually settling in Meridian, Scott worked at an urgent care where he handled minor emergencies, fracture management, and laceration repair among other physician assistant duties.

Scott's personal philosophy of quality instruction with compassion allows him to provide personalized care for patients of all ages and backgrounds. As an avid outdoorsman, Scott enjoys hiking and camping where he loves to capture his beautiful surroundings with his camera. He is a black belt in Tae Kwon Do and plays jazz string bass in his spare time. His wife and five boys are his life, and he is excited to work at Meridian Family Medicine where he can build relationships with patients and do what he loves.

Janelle joins our practice from Arizona. She attended Arizona State University for both her RN and NP training and graduated with honors. She has over 13 years of experience as a family nurse practitioner and over 16 years as a registered nurse with varied areas of experience.

She worked in pediatrics, step-down NICU, and postpartum as a registered nurse, and has been caring for all ages in family practice as a nurse practitioner.

Janelle believes her role as a provider is to be a partner and advocate for her patients. She is proficient in skin procedures, joint injections, trigger point injections, and is passionate about helping her patients achieve a healthier lifestyle through comprehensive medical weight loss. Some of her areas of specialty are treating migraines, thyroid disease, chronic fatigue, diabetes, and weight loss assistance.

Janelle loves being a wife and mom to her four kids at home. She loves reading, traveling, and going on adventures with her family. She loves the four seasons and beauty of Idaho.

Jami joins our practice with over 18 years' experience as a Family Nurse Practitioner. She graduated with her NP degree from Seattle University in 2002 and has loved working in the field of family medicine for most of her career.

Jami enjoys the long-term patient relationships that can be formed in primary care and enjoys the variety of caring for people of all ages, with a particular interest in adolescent and women's care. She values the importance of lifestyle choices in preventing disease and maintaining lifelong health.

Jami moved to Idaho in 2020 after living 25 years in Washington State. She and her husband have 2 children and love the beautiful weather and the active, outdoor lifestyle that Idaho has to offer.

Nicole Thomas, PA, is a committed and compassionate healthcare provider who has been practicing medicine for over five years. She has a strong passion for nutrition and fitness, which fuels her dedication to preventing chronic disease through sustainable, healthy lifestyle choices. Nicole focuses on building trusting relationships with her patients, believing that informed decisions come from open dialogue and mutual respect.

An Idaho native raised in the Treasure Valley, Nicole earned her degree from Idaho State University and is proud to serve the community she calls home. She lives in the area with her spouse and their two children, balancing a busy home life with an active lifestyle. When she's not seeing patients, you can often find her supporting her kids in jiu-jitsu and football. A lifelong learner and fitness enthusiast, she also loves baking, gardening, traveling, and camping.
